SDOH Accelorator Plan
On Sept. 30, 2021, the Center for Disease Control (CDC) awarded Neighbors Building Neighborhoods (NBN) a $125,000 grant to conduct a Social Determinants of Health (SDOH) Accelerator Plan for Muskogee County. NBN was 1 of 20 recipients in the entire nation and is serving as a facilitator for the Muskogee County Social Determinants of Health Consortium (MCSDOHC) to discover strategies to meet health equity needs for the residents of Muskogee County. The MCSDOHC is now collaborating with local governments, the county health department, healthcare providers, tribes, businesses, and community-based organizations to identify health needs specific to Muskogee County. Using data-driven, evidence-based methodologies the consortium will develop a plan for the implementation of strategies that can improve the wellness of low income and rural residents of Muskogee County and reduce health disparities by positively impacting social determinants of health (SDOH).
Upon completion of the SDOH Accelerator Plan, St. Francis Health Systems has already committed $100,000 toward the implementation phase to serve Muskogee County. Please fill out our community survey to help.
